Introduction

Please use this form to report any actual or suspected information risks, issues or concerns that have not been previously reported or identified.

This form is NOT for the purposes of whistleblowing or anti-bribery. Please follow your Department’s procedures to report on these issues.

It is extremely important that we identify, contain, manage and resolve any risks, issues or concerns affecting government information as quickly and professionally as possible.

In certain circumstances the government is obliged by law to report some of these events and to do so within a finite time period. In order to comply with this requirement, the more detailed the information is when first reported, the better – BUT this should not delay communicating the concerns at the earliest opportunity.

Important notes

  1. Do NOT include any personal or sensitive information in your report except for contact details. If further investigation is required then you will be contacted directly by authorised personnel investigating your report.
  1. If providing contact details other than your own, please ensure you have permission to share.
  1. If your risk, issue or concern involves technology, the report will be forwarded on to Government Technology Services (GTS) or relevant technology service provider for information or to take action where required.

What will happen when you submit a report

Your report will be sent to authorised personnel in the affected Department or business area. They will review, investigate and, if required, work to mitigate and resolve any issues.
