Introduction

This form is to apply for a registration of a business and person(s) to carry out acupuncture, tattooing, microblading, body piercing and electrolysis.
 
Please note:
 
  • a fee of £100 per application is payable prior to an inspection being undertaken
  • a separate application is required for each individual working at any given premises i.e. an individual would need to register at each premises that they will be carrying out business
  • a separate application must be made for each person to be registered

 

Process and supporting documentation

This application form when completed will be sent to the Environmental Health Unit.

The completed application form must be accompanied by a plan, drawn to a suitable scale of not less than 1:100, showing the internal layout (including facilities) and position, within the buildings, of the premises for which registration is applied, and clearly defining, in red, that part of the premises to be used as the treatment area. This should include the position of the wash hand basin (please note it is not acceptable for the wash hand basin to be in a different room).

Once received and processed, your application will be allocated to an Environmental Health Officer, who will contact you to arrange for an inspection of your premises to be carried out. Please note that, as part of the inspection, you may be asked to provide evidence of the following:

  • First Aid Training
  • Completed training and any further training to be undertaken
  • Public Liability Insurance
  • Written pre and post treatment advice
  • Blank client medical questionnaire/consent form
  • Blank parental consent forms for anyone under the age of 18 (body piercing only). It is an offence to perform any other procedures listed on a person under the age of 18, unless you are medically qualified

If, at the time of the inspection, you are unable to provide any of the above information, the Officer may be required to delay or prevent registration.
